Roll v0 history into docs/ARCHIVE.md; retire CONTEXT.md, CONTEXT-ARCHIVE.md,
PLAN.md, COMPLETED.md. Move plan docs under docs/. Rescue 9 live deferrals
into docs/TODO.md.
